Summer Band Registration

Step 1: Read and print out this letter and payment form.

Step 2: Fill out this online registration form. Click "Submit" at the end. You will receive an email confirmation.

Step 3: Complete the bottom portion of the form in Step 1.

Step 4: If you chose to pay by check, attach your check (made out to West Lafayette Band Boosters) to the form.

Step 5: If you chose PayPal as your payment method, use the following button to pay. Once you are at the PayPal site, enter the amount you chose and proceed to payment. Please note that although Band Boosters gets a charity rate, a payment of $100.00 still incurs a fee of $2.50, which Band Boosters will have to absorb. Therefore, if users of PayPal can add a small donation to their payment to cover the fee, that would be very much appreciated.



Step 6: Turn in the form and payment to Mr. Pettit or Mr. Henady by Friday, May 19, 2017.
